Yosef goes home after a stroke

How a rehabilitation ward turns its discharge plan into daily steps at home, and sees how Yosef is doing before the follow-up visit.

This person and story are fictional. The screens are from the TailorPath app with sample data, so names and details on them differ from the story.

Meet Yosef, 67

A retired electrician, recovering from a stroke on a rehabilitation ward. He lives with his wife; his daughter Rina lives nearby. He wants to walk to the synagogue on his own again.

Goals they set together

  • Walk 100 metres indoors with a cane, without help
  • Daily balance and walking practice at home
  • Get in and out of a chair safely

The plan, step by step

  1. Step 1The rehabilitation team

    Start from the ward’s template

The physiotherapist opens the ward’s post-stroke walking template instead of a blank page. Exercises, reminders and questionnaires the team already trusts are filled in.

Plan templates in the clinician web app (sample data)
  2. Step 2The rehabilitation team

    Set goals Yosef cares about

Before discharge, they agree on goals with Yosef. Each goal has five levels, from −2 to +2, so “walking better” becomes something the team can score at follow-up.

Plan details: a walking goal with its five goal-attainment levels (sample data)
  3. Step 3Yosef

    His day at home

At home, Yosef sees today’s exercises with their times, gets a reminder and marks each one done. Text can be made larger with one tap.

Regular layout: today’s missions on the phone (sample data)
  4. Step 4Rina, his daughter

    The family takes part

Rina answers a short caregiver questionnaire each week, as the plan shares with her. The team sees how things look from home, not only in the clinic.

Questionnaire list on the phone, with assigned and submitted questionnaires (sample data)
  5. Step 5The rehabilitation team

    Between visits

The dashboard shows which questionnaires are waiting and which plans need an update, so the team can call before a small problem becomes a readmission.

Clinician dashboard: assigned people, questionnaires and today’s schedule (sample data)
  6. Step 6The rehabilitation team

    Follow-up, with evidence

At the follow-up visit, the team scores each goal and sees the trend since discharge, next to how many exercises Yosef completed.

Plan statistics: goal attainment over time and questionnaire answers (sample data)

At the 12-week review

In this story, Yosef walks indoors with a cane on his own, and the team moves his plan to the next goal: walking outdoors to the synagogue with Rina.
