Meet Amir, 21

A young autistic man who likes trains and clear routines. He is moving from his parents’ home into a supported-living flat and wants to cook simple meals and travel to his day programme on his own.

Goals they set together

  • Make breakfast on his own, following picture steps
  • Take the bus to his day programme
  • Call his parents twice a week

The plan, step by step

  1. Step 1His case manager

A plan from the service’s template

The case manager starts from the service’s daily-living template and adapts it with Amir and his parents to his routines and interests.

  2. Step 2Amir

    The layout that suits him

Amir chose the colorful layout: big tiles with pictures, and every screen can be read aloud.

Colorful layout: the app menu as large picture tiles (sample data)
  3. Step 3Amir

His day, one step at a time

Each morning he sees what is next, with the time and a picture. When he finishes a mission, he taps End and the next one comes up.

Colorful layout: today’s missions with times and progress (sample data)
  4. Step 4Support staff

Staff work from the same missions

Support staff on each shift work from the same missions in the web app. They see how many are completed, active or missed, and add a new mission when Amir is ready for the next step, instead of passing notes between shifts.

Missions in the clinician web app: totals of completed, active, failed and assigned missions (sample data)
  5. Step 5His parents

    Parents as partners

His parents answer a monthly questionnaire and can message the team. They see what their role in the plan allows, and Amir’s privacy is respected.

Questionnaire list on the phone (sample data)
  6. Step 6The professional team

Progress the whole team can see

At the quarterly meeting, the team, Amir and his parents look at each goal’s score over time and agree on the next step.

Plan statistics: goal attainment over time (sample data)

At the quarterly meeting

In this story, Amir makes breakfast on his own most days and takes the bus with a staff member nearby. The next goal is the bus ride on his own.
